What is it?

Phacoemulsification is a surgical procedure where a natural lens is emulsified with an ultrasonic device and then aspirated from the eye. Then the surgeon places a foldable Intra Ocular Lens (IOL) in its place to provide a clear vision.

Phacoemulsification is a modern extracapsular cataract extraction surgery that will help in removing the lens fibers that form the nucleus and cortex of the cataract. This will leave the posterior epithelial capsule to hold the new artifical IOL.

Phacoemulsification can also be defined as an extracapsular surgical technique that is performed to extract the cataract.

When is it needed?

Phacoemulsification, which is usually involved in a cataract surgery with IOL implantation, is most commonly done to improve a patient’s vision.

In most cases, it would be the patient’s desire to overcome the minimum vision standards set by government agencies for driving licenses, flying, or to operate complex equipment.

Before Procedure

The patient will be placed in the temporal position where the patient lies supine or on is/her back on the surgical table.

The doctor will then administer the following anesthesia as drops:

  • Topical Anesthesia: helps block the creation of the pain sensation. However, it will not block the transmission of the pain by not suppressing the motor, thermal and tactile fibers of the nerve. It usually consists of analgesics.

Once the anesthetic is administered, an antiseptic solution is scrubbed on the eyelashes, and then a transparent steri-drape is placed on the eye to move the eyelashes out of the way.

Procedure

Phacoemulsification, which is usually involved in a cataract surgery with IOL implantation, is most commonly done to improve a patient’s vision.

The surgeon will then create a 2.5mm incision along with tiny incisions on the side to insert the surgical instruments. Then the anterior face of the capsule containing the lens present inside the eye is removed.

The Phalcoemulsification machines have parameters for power, vacuum, pulse, burst, and oscillation levels that are set to allow certain phaco procedures to be done during the phacoemulsification surgery.

The surgeon will then use an irrigation/aspiration device to remove the residual cortex. The surgeon will start the implantation process loading the foldable IOLs on to a cartridge before injecting into the capsular bag. This reduces the risk of the eye being exposed to any contaminants.
